ONLINE APPLICATION
SUBMISSION PROCESS
PLEASE REVIEW CAREFULLY
The NATA Research and Education
Foundation offers an opportunity for committed
athletic training students to obtain financial assistance for the cost of their
education. The application process is now online. Applicant may enter the online system
multiple times to review and finalize application. However, all Scholarship
material MUST be submitted prior to Friday, February 5, 2010.
Please review the following information
carefully before entering the Scholarship Application Submission Area.
General Information
1. Three (3) scholarship application categories are
currently available. Once a scholarship has been awarded to an individual, s/he
is no longer eligible for a scholarship in the same category.
a. Entry Level
– Undergraduate OR Master’s
b. Master’s
c. Doctoral
2. NATA Membership is a requirement (NATA
Membership)
3. All NATA
Foundation scholarship awards for 2010 are $2,300.
4. Need
shall not be a factor in granting NATA Foundation scholarships. Assistance from
other sources will not make the applicant ineligible for an NATA Foundation
scholarship. Consideration will also be given to the applicant’s participation
in campus activities other than academic and athletic training, in which s/he
has demonstrated qualities of leadership and has been a positive example to
fellow students.
5. Applicants
will be notified of the Scholarship Committee’s decision by April 15, 2010.
6. The
selected applicant must provide proof of enrollment in an appropriate undergraduate,
entry level master’s, master’s or doctoral
program for the 2010/2011 academic year before a check will be
issued. Proof of enrollment can come from a variety of University sources, but
must be official documentation. The recipient is expected to remain enrolled as
a full time student in that program. NOTE: An exception may be granted if the
recipient is assigned to active military service or due to other extenuating
circumstances acceptable to the NATA Foundation Scholarship Committee.
Eligibility Requirements
A. The Entry
Level (Undergraduate or Master’s) Scholarship applicant must:
i.
Apply no later than November 1, 2009 to become a NATA
member.
ii.
Have a cumulative
overall GPA of at least 3.2 (based on a 4.0 maximum) for ALL undergraduate
courses and ALL graduate courses (if applicable). In order to qualify,
all applicants MUST have a minimum of
5 semesters or 7 quarters of college course work completed including the Fall of 2009.
iii.
Be enrolled in a CAATE accredited
undergraduate program requiring four years for a baccalaureate degree,
may apply during his/her junior year, or immediately prior to his/her final undergraduate
academic year (provided that the submission due date requirement is met)… OR
… be enrolled in a CAATE accredited entry level master’s program and
may apply during their first year of entry level graduate study.
iv.
Have performed with distinction as a participant in
his/her athletic training program, academic major, institution and
intercollegiate athletics.
v.
Confirm his/her intent to pursue the athletic
training profession as his/her primary means of livelihood
B. The Master’s Scholarship applicant must:
i.
Have been a member of NATA since February 5, 2009, or earlier.
ii.
Have a cumulative
overall GPA of at least 3.2 (based on a 4.0 maximum) for ALL undergraduate
courses and ALL graduate courses including the Fall of
2009.
iii.
Be or plan to be, enrolled in a master’s degree
program, and may apply during his/her final academic year of undergraduate work,
or anytime prior to his/her final academic year of master’s work
(provided that the submission due date requirement is met).
iv.
Have performed with distinction as a participant in
his/her athletic training program, academic major, institution and
intercollegiate athletics.
v.
Confirm his/her intent to pursue the athletic
training profession as his/her primary means of livelihood.
C. The Doctoral Scholarship applicant must:
i.
Have been a member of NATA since February 5, 2009, or earlier.
ii.
Have a cumulative
overall GPA of at least 3.2 (based on a 4.0 maximum) for ALL graduate courses
including the Fall of 2009.
iii.
Be or plan to be, enrolled in a doctoral degree
program, and may apply during his/her final academic year of master’s work,
or anytime prior to his/her final academic year of doctoral work
(provided that the submission due date requirement is met).
iv.
Have performed with distinction as a participant in
his/her athletic training program, academic major, institution and
intercollegiate athletics.
v.
Confirm his/her intent to pursue the athletic training
profession as his/her primary means of livelihood.
Application Information
All sections (Sections I-V) of the
scholarship application MUST be
fully completed and submitted as directed.
This MUST be completed at initial logon to
system.
1.
Section I: There are three (3) parts
to this section.
a. Applicant’s Information. Includes contact information and NATA member number and allows
candidate to select scholarship category.
b. Verification of intent to pursue the athletic training profession. You will be asked to verify the following statement: “ I intend to pursue athletic training as my primary means of livelihood”.
c. Contact Information for Institutional
Representative. This individual verifies enrollment in
appropriate program based on category of scholarship chosen.
i.
Please include name and credentials,
title, phone number, and email.
Acceptable Institutional Representative:
1.
For Entry
Level Undergraduate – Program Director or Clinical Education Coordinator.
2.
For Entry
Level Master’s – Program Director or Clinical Education Coordinator.
3.
For Master’s
or Doctoral – Department Chair or Thesis/Dissertation Chair.
d. Contact Information for Nominating ATC. This individual is a BOC certified athletic trainer,
who is a member in good standing of NATA, and who can attest to the applicant’s
skills, abilities and scholarly activities as they relate to the applicant.
He/She has agreed to write a letter of support for
your application.
i.
Please include name and credentials,
title, phone number, and email. NOTE: This
may be the same individual who
served as Institutional Representative.
A
fully completed application packet, to include Sections I through V,
must be received by the NATA Foundation no later than
February 5, 2010, for the scholarship applicant to be eligible
for consideration for a scholarship award.
2.
Section II: Application for
Scholarship includes:
a. Complete academic information (includes all universities
attended),
i.
Candidate MUST send official academic transcripts from each university to: NATA
Foundation, 2952 Stemmons Freeway #200,
b. Athletic training experience,
c. Awards and Activities,
d. Applicant’s Essay. Please address each of the components suggested
in essay instructions.
3.
Section III: Completed Institutional
Representative Form. Submitted on-line by identified representative. You
will be able to log-on and see when it was submitted.
4.
Section IV: Completed Nominating ATC
Letter Form. Submitted on-line by identified nominator. You will be able to
log-on and see when it was submitted.
5.
Section V: Applicant's Confirmation. This
is your verification that you have completed all of Section II and have
follow-up or secured an Institutional Representative endorsement and Nominating
ATC endorsement.
HELPFUL HINTS
1. If you have begun (but not completed) your scholarship application
and need to save it and return to it later, you must enter something into EACH
field. For example, if you have not completed your essay, simply put a question
mark or some other character into the essay field in order to be able to save
your application until you are ready to begin working on it again.
2. Your Nominating ATC and Institutional Representative must be two
different individuals. One individual cannot serve as your Nominating ATC and
your Institutional Representative. Also, please make sure that these
individuals are aware of your application, and have AGREED IN ADVANCE to
complete the necessary forms.
3. There are three scholarship categories: (1) entrylevel/entrylevel
master's, (2) master's and (3) doctoral. Please make sure that you complete the
correct application for your level.
4. Transcripts will be required as part of your application. You are
required to provide your official transcripts via mail.
5. Please call the NATA Foundation office (1-800-879-6282 ext.121) with any questions. A
fully completed application packet, to include Sections I through V,
must be received by the NATA Foundation no later than
February 5, 2010, for the scholarship applicant to be eligible
for consideration for a scholarship award.
